What to Bring to Your Appointment - Your insurance card
- Physician referral forms, if required by insurance
- List of prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent x-rays or relevant records

Forms to Bring With You
Please print and complete the 4-page questionnaire and bring with you to your first appointment and contact our office if the form is completed at home. If the form is completed at home, you will only have to come in for your first office visit fifteen minutes prior to your scheduled appointment instead of thirty minutes prior to your first visit. This is only if the 4-page questionnaire is completed prior to your first appointment.

Insurance and Payment Information Allergy Associates & Lab., Ltd. is a provider for Medicare and most major insurance plans, and provides insurance billing. You are responsible for charges that are not covered by your insurance, at the time of your appointment.
Your insurance company may require a co-payment at the time of your appointment. When necessary, our staff will work with patients to set up a payment plan.
